Your first Botox appointment can feel like a significant step — especially if you are not entirely sure what to expect, whether it will hurt, how long the results will last, or whether you will still look like yourself afterward. These are all valid questions, and they are exactly the kinds of things we address every day at The Modern Woman Med Spa in Santa Rosa.
This guide walks through the fundamentals of Botox — how it works at a biological level, what your first visit looks like step by step, what results to expect and when, common myths that need correcting, and what to know beforehand so you feel informed, confident, and comfortable.
What Botox Actually Does at the Muscle Level
Botox — the brand name for onabotulinumtoxinA — is a purified protein derived from Clostridium botulinum. When injected in very small, precise doses into specific facial muscles, it temporarily blocks the release of acetylcholine, the neurotransmitter that signals muscles to contract. Without that chemical signal, the targeted muscle relaxes.
By relaxing the muscle, the overlying skin smooths out. The dynamic wrinkles — the lines that form when you squint, frown, raise your eyebrows, or smile — become softer and less visible because the repetitive movement that created them is temporarily reduced.
The most commonly treated areas include:
- Forehead lines: The horizontal creases that appear when you raise your eyebrows. These are caused by contraction of the frontalis muscle
- Glabellar lines (the “11s”): The vertical furrows between your eyebrows caused by the procerus and corrugator muscles. These lines can make you look frustrated or tired even when you feel neither
- Crow’s feet: The fine lines that fan outward from the outer corners of your eyes, caused by the orbicularis oculi muscle when you smile or squint
- Bunny lines: The small horizontal creases on the sides of the nose
- Chin dimpling: The pebbly, “orange peel” texture on the chin caused by overactivity of the mentalis muscle
An important distinction: Botox does not fill wrinkles or add volume (that is what dermal fillers like Juvederm or Restylane do). Botox prevents the muscle movement that creates those lines in the first place. The result, when done well by an experienced injector, is smoother skin that still allows natural expression and emotional communication.
Preparing for Your First Appointment
There is not an extensive amount of preparation required for a Botox appointment, but several practical steps help ensure the best possible experience and outcome.
Avoid Blood-Thinning Substances
If possible and medically appropriate, avoid aspirin, ibuprofen (Advil, Motrin), naproxen (Aleve), fish oil supplements, vitamin E, and excessive alcohol for approximately one week before your appointment. These substances thin the blood and can increase the likelihood of bruising at the small injection sites. Important: if you take prescription blood thinners such as warfarin or apixaban, do not discontinue them without explicit guidance from your prescribing physician. Simply let us know during your consultation.
Skip Alcohol the Night Before
Alcohol dilates blood vessels and increases blood flow to the surface of the skin, which raises the chance of post-injection bruising. A night of good hydration and rest is genuinely the best preparation you can do.
Arrive with Clean Skin
We will cleanse the treatment area thoroughly before beginning, but arriving without heavy foundation, concealer, or setting powder on the areas you want treated helps streamline the process and reduces the time spent on preparation.
Know What Bothers You
Spend a few minutes before your appointment looking at your face in natural light — not the harsh lighting of a bathroom mirror. Think about which lines you notice most. Do the forehead creases bother you? Are the lines between your brows creating a permanent expression you do not feel? Are the crow’s feet more prominent than you would like? Having a clear sense of your priorities helps us create a treatment plan that aligns precisely with your goals.
What Happens During Your Botox Appointment
A first Botox appointment at The Modern Woman Med Spa in Santa Rosa is straightforward and efficient. The entire visit, including the consultation portion, typically takes 20 to 30 minutes from start to finish.
Your Consultation
Dr. Shazah Khawaja, Board-Certified OB-GYN, or a member of our trained clinical team will sit with you in a private treatment room. We will examine your facial anatomy at rest and during natural expressions — asking you to raise your eyebrows, frown, squint, and smile so we can observe your muscle patterns and identify the specific muscles driving the lines you want to address.
This assessment is important because good Botox treatment is not about erasing every line or freezing every muscle. It is about creating a natural, refreshed appearance that reflects how you feel on the inside. We discuss your goals, your comfort level (some patients want a subtle softening; others want more significant smoothing), and what realistic results look like for your specific anatomy.
Treatment Mapping
We may mark small dots on your face with a gentle marker to indicate the precise injection points. These points are customized to your unique facial anatomy. Every face is different — the width of your forehead, the strength and mass of your frontalis muscle, the distance between your brows, the depth and pattern of your crow’s feet — and your treatment should reflect those individual characteristics rather than following a generic template.
The Injections
Botox is administered using an extremely fine needle — 30 or 32 gauge, which is much smaller and thinner than the needles used for blood draws or vaccinations. Most patients describe the sensation as a brief, tiny pinch at each injection point — mild, quick, and tolerable without any anesthesia. For most treatment areas, we use three to five small injections per zone to distribute the product evenly and symmetrically.
The entire injection process — from first poke to last — takes about five to ten minutes. Many patients are surprised at how fast it goes.
Immediately After
You can walk out of the office and return to your normal daily activities right away. There is no downtime, no bandages, and no visible signs that anything happened — aside from small red bumps at the injection sites that typically resolve within 15 to 30 minutes. Occasional minor bruising at one or two injection points is possible and is easily covered with makeup if it occurs.
When Results Appear and How Long They Last
Botox does not work instantaneously. The protein needs time to bind to the nerve receptors and interrupt the acetylcholine signal. Here is the typical timeline most patients experience:
- Days 1 through 3: You may not notice any visible change. The product is beginning to take effect at the neuromuscular junction, but the muscles have not yet relaxed enough to show a difference
- Days 4 through 7: You will start to notice reduced movement in the treated muscles. Lines may begin to soften. The treated areas may feel slightly different when you try to make expressions
- Days 10 through 14: Full effect is typically visible by the two-week mark. This is when you will see the smoothest, most balanced results
If this is your first treatment, we frequently recommend a conservative initial dose. We can schedule a brief two-week follow-up to evaluate the results and add a few additional units if needed. This approach is safer and more reliable than using too much product on the first visit — because once Botox is in, you cannot remove it. You simply wait for it to wear off. Starting conservatively and adding strategically is always the wiser approach.
Results typically last three to four months for most patients. As the Botox metabolizes and clears from the nerve endings, muscle movement gradually returns and the dynamic lines will reappear. With consistent treatment over time — typically every three to four months for the first year or two — many patients find they need fewer units or less frequent sessions because the muscles become trained to contract with less force.
Addressing Common Myths and Legitimate Concerns
Will I Look Frozen or Expressionless?
Not if it is done correctly and by an experienced injector with a conservative philosophy. The “frozen” appearance that gives some people pause is the result of too much product, poor placement, or an aggressive treatment philosophy. At The Modern Woman Med Spa, our approach prioritizes natural movement and preserved emotional expression. We want your face to move — just with less of the repetitive muscle force that creates deep lines.
Is Botox Safe?
Botox has been FDA-approved for cosmetic use since 2002 and has been used therapeutically in medicine since the late 1980s for conditions including strabismus, cervical dystonia, and chronic migraines. It has one of the longest and most extensively studied safety records of any cosmetic treatment. Side effects are generally mild and temporary — most commonly minor bruising, a slight headache on the day of treatment, or temporary tenderness at the injection site.
Will It Be Painful?
Most patients rate the discomfort as a 2 or 3 on a scale of 10. The needles used are extremely fine, and each injection takes less than a second. If you are particularly sensitive to needles or have anxiety about the process, we can apply a topical numbing cream or ice to the treatment area beforehand. Most first-time patients report being pleasantly surprised by how quick and tolerable the experience is.
Am I Too Young (or Too Old) for Botox?
There is no single “right age” to start. Many women in their late 20s and early 30s begin Botox as a preventive strategy — sometimes called “baby Botox” — using small amounts to relax muscles before deep, static lines have a chance to form. Women in their 40s, 50s, and beyond use Botox to soften existing lines and maintain a refreshed appearance. The appropriate approach depends on your anatomy, your skin, and your goals — not a number.
Post-Treatment Guidelines for Best Results
To ensure optimal results and minimize any risk of the product migrating from the injection sites, follow these guidelines for the first 24 hours after your treatment:
- Do not rub, press, or massage the treated areas of your face
- Remain upright for at least four hours after treatment — avoid lying flat or bending forward extensively
- Skip intense exercise, heavy lifting, and activities that significantly raise your blood pressure for 24 hours
- Avoid saunas, steam rooms, hot tubs, and hot yoga for 24 hours — excessive heat can increase blood flow and potentially affect product distribution
- Do not wear hats, headbands, or goggles that press firmly against the treated forehead area
